During the Fall semester a laptop which contained all of the CAP CSI's personal information was stolen.  We urge you to periodically check your credit report to make sure that your identity has not been stolen.  Please refer to the links below regarding information on identity theft.  This does not apply to any of you who applied this Spring 2008 semester.

-Participant Registration Form (required of all CURRENTLY ENROLLED CAP participants)

Payroll Matters:

-CAP Instructor's Timesheet (see instructions on how to complete below)**Next timesheet due date for pay period April 28-May 11, 2008 is due on Wednesday, May 7, 2008 by noon and will be paid on May 16, 2008.  Any timesheets received later than this date will not be turned in until the next timesheet due date of Wednesday, May 20, 2008.  If you would like to have your paycheck mailed to you, you must submit a self-addressed, stamped envelope to the payroll office before May 16, 2008.

-CAP Payroll Schedule **Next timesheet due date for pay period April 28-May 11, 2008 is due on Wednesday, May 7, 2008 by noon and will be paid on May 16, 2008.   If any timesheets are received later than the due date, they will be submitted with the next pay period and will be paid on the following pay date.  Please refer to the payroll schedule.

-Reimbursement Form (use if reimbursements are over $40 total)

 

Payroll Forms for Employment:
All forms below are mandatory before you begin working with CAP
-Application for Employment (for non-CalArts students)
-Employee Data Form (for non-CalArts students)
-Form I-9
-Form W-4

-Copies of I.D. (See list of acceptable documents under I-9 form)

**If payroll forms are not received and you have started working with CAP, then you will be forced to stop working until all forms are received.**
